The National Fragility Index, launched by the API Innovation Center, warns that more than 550 million prescriptions filled in the U.S. in 2023 were for drugs ranked as having high or severe vulnerability to potential supply chain disruptions.
The index, which debuted Tuesday and is produced by APIIC in conjunction with industry supported market intelligence and supply chain platform QYOBO, was designed to provide detailed tracking of pharmaceutical supply chain risks in the United States.
The inaugural NFI study focused on more than 400 drugs, covering 131 treatment categories. Fourteen were rated severe and 71 as high fragility, which combined represents one in five medicines facing supply-chain stress, the group said in a press release.
Almost 6 in 10 of the drugs analyzed by the NFI have experienced at least one shortage, with the average length of those shortages lasting more than two years, APIIC added.
Two drugs highlighted in the report were pantoprazole, a widely used stomach-acid medicine with 34 million prescriptions filled in 2024, and metoprolol, the most prescribed systemic beta-blocker in the U.S., with more than 58 million prescriptions filled in 2024.
Pantoprazole was rated severe on the index, which found that just Just 5.6% of the drug dispensed domestically comes from a U.S.-based finished-dose manufacturer. It has reported shortages five times since 2015.

Metoprolol, which was rated high, had 58 million scripts filled in 2024. The index found there is no domestic source for APIs used to produce the drug, which has experienced two shortages since 2015, with each shortage lasting an average of nearly five years.
According to the Administration for Strategic Preparedness and Response, at least 11 of the 86 essential medicines have a U.S. manufacturer, however, none are currently supplying U.S. drugmakers.
“We cannot know whether our investments are strengthening the drug supply unless we can measure the risks they are meant to address,” said Natalie de Graaf, VP and general manager for data analytics, AI and machine learning solutions at St. Louis-based APIIC, in a statement. “The National Fragility Index gives decision-makers a starting point. It helps them see which supply chains need attention today and track whether their choices are making a difference.”
The index comes in the wake of the COVID-19 pandemic that exposed problems in the global pharma supply chain, with pushes by both the Biden and Trump administrations to “on-shore” more drug manufacturing in the U.S. Its focus is to spot problems in order to pivot supply chains for national security and bolster U.S. drug manufacturing capabilities, APIIC said.
“For years, pharmaceutical companies have used our supply chain intelligence to make decisions with confidence,” Markus Felgenhauer, co-founder and CEO of QYOBO, said in a statement. “We’re excited to bring that same intelligence to the public sector, giving policymakers a clear view of supply dependencies and manufacturing sites around the globe, so they can direct resources where they will strengthen the resilience of the U.S. drug supply and ultimately benefit American patients.”
